Award Rules
Please note: Deadline to meet criteria and submit forms is April 22, 2010.
All the following criteria must be met to receive recognition. Please include supporting documentation.
 |
Team has at least 10 registered team members one month before event. Please attach roster from online site or provide hand-written list. |

Team Captain to host at minimum 1 team meeting one month before event. Please attach date of meeting along with sign in log from meeting. |
|
|
All Team Members have raised $100 each before submitting this application. Confirmed thru Online fundraising or Fundraising envelopes submitted to Accounting. |

|
Fight Back Message/Activity or On-Site Fundraiser Planned For Event Please attach description of theme/activity or fundraiser planned. |
|
|
Team has at least 1 ACS CAN member or Committee Member on team. Please note name(s) of team members and role. |
The Team will receive a commemorative “Spirit Stick” and recognition at local event.
