qualify for the Trailblazer Team Award by
completing the checklist of requirements listed below. It's really easy--ALL

Award Rules
Please note: Deadline to meet criteria and submit forms is April 22, 2010.
All the following criteria must be met to receive recognition. Please include supporting documentation.

Team has at least 10 registered team members one month before event.
Please attach roster from online site or provide hand-written list. |
|
Team Captain to host at minimum 1 team meeting one month before event.
Please attach date of meeting along with sign in log from meeting. |
|
All Team Members have raised $100 each before submitting this application.
Confirmed thru Online fundraising or Fundraising envelopes submitted to Accounting. |

Fight Back Message/Activity or On-Site Fundraiser Planned For Event
Please attach description of theme/activity or fundraiser planned. |
|
Team has at least 1 ACS CAN member or Committee Member on team.
Please note name(s) of team members and role. |
The Team will receive a commemorative “Spirit Stick” and recognition at local event.
