Registering by Mail The easiest way to register for Relay For Life is through our American Cancer Society Web Site. Just follow the prompts and fill in the requested information. A credit card is not necessary unless you would like to make a donation. However, if you would like to register by mail, please follow the instructions below. Team Registration To register a Team for Relay For Life, please complete a Team Registration Form and mail it to the address below. For more information, contact our Team Captain Coordinator, Stacy Kupfer at Teams@scvrelay.org. Survivor Registration To register for Relay as a Survivor, please complete our Survivor Registration Form and mail it to the address below.  You may join with friends and family members as a Team or as an individual.  For more information, contact our Survivor Chairs, Cheryl Adams and Cassie Taylor, at Survivors@scvrelay.org.
